| dc.description.abstract | This qualitative study examined clinicians on Chronic Disease Teams in Nova Scotia‘s perspectives of self-management. Findings indicate that they often view self-management as an individual journey. They believe that effectively working with self-management involves considering what is important to patients, their coping strategies, and difficulties they may be encountering. | |
